Nursing Applications & Renewals


Nursing applications and renewals are processed online via OPR’s Online Services System - log in to apply, renew, update an application/license, check your status, request a verification, and more.

Nursing license renewal is not contingent upon a completed background check. Fingerprint-supported background checks are required for nursing licensure in Vermont. If you have done a criminal background check for OPR nursing licensure and have not had a lapse in licensure you DO NOT have to complete another criminal background check. Learn more about Vermont’s new criminal background check requirements.

A 90-day provisional license can be issued if all application requirements are complete EXCEPT for the official verification of licensure or the results of a criminal background check. Apply online and request your 90-day temporary license today.

Certain healthcare professionals licensed in another state can provide telehealth services to Vermonters or work in a Vermont facility. Please visit our Telehealth & Out of State webpage for more information.

If you have been licensed in another state for 3 or more years, you may be able to apply for accelerated licensing. Please visit our Fast Track Endorsement Webpage for more information.

Vermont APRNs are required to hold either a Vermont RN license OR hold a compact (multistate) RN license in a compact state.

Continuing Education Information

Continuing education hours will be accepted from accredited Continuing Education providers or programs on topics related to medication administration, medication safety, medication documentation, and medication error prevention.

From OPR to another licensing authority

Please log in to your online services account to request a verification of licensure. There is a $20 fee. OPR offers official online verification at no cost at Find a Professional.

The below forms can be used but are not required. Every state is different, please contact the receiving state. Verifications are processed within 5 business days.

To OPR from another licensing authority

Verifications must be sent directly to OPR from the licensing authority. These may be sent by U.S. Postal Mail or by email to SOS.OPRLicensing1@vermont.gov

Verifications are processed within 5 business days after receipt.
